Perry Health has partnered with Zing Health to help you better manage your Diabetes. The Perry Health team will not replace your PCP or Endocrinologist but will offer additional care and support needed between visits.
As a patient of Perry Health, you will receive monthly telephonic personalized care. Your dedicated team will consist of coaches, dieticians, and nurses who specialize in Diabetes management. Your care plan is coordinated with your PCP and other care providers to ensure a seamless experience.

Important Message About What You Pay for Vaccines:
Our plan covers most Part D vaccines at no cost to you, even if you haven’t paid your deductible (if applicable). Call Customer Service for more information.

You won’t pay more than $35 for a one-month supply of each insulin product covered by our plan, no matter what cost-sharing tier it’s on, even if you haven’t paid your deductible (if applicable).
